Ministry Applications
Each volunteer in Children's Ministries completes a new Ministry Application as well as undergoes a background check. There are two different forms:
Ministry_Application.pdf is for any adult who is or would like to serve in Children's Ministries.
Ministry_Application_for_Leaders_in_Training.pdf is for anyone under age 18 adult who is or would like to serve in Children's Ministries.
Please turn in the completed form(s) to the church office. If you are unable to print the applications, you can get copies at the church office. The cost of the background check is $10.00 per person. If you are able to help offset the cost of this for your application, please attach a check to your application when you turn it in to the church office.
